And Indy wasn't even on a boat.
She is normally a mischievous
four-year-old papillion cross with hairy pointed ears.
(Papillions are wispy-haired, fine-boned little dogs
of about 4kg, with a placid temperament.) She had
been vomiting in the morning and was very subdued
by the afternoon.
As I carried her over to the scales to weigh her,
she suddenly vomited on to the floor what looked
to be a litre of curdled milk, which gave the whole
waiting area a look like those children's spinning
paint-splatter paintings. Indy's dad
Ian explained that he thought the milk
might settle her stomach. What a mess!
Vomiting is a common complaint for my
patients and we see cases almost every
And everyone asks, "what's wrong with her?"
Basically, it can be almost anything you can think of.
And to make matters worse, they can't tell us and we
haven't got a crystal ball.
We need to have a step-wise approach to finding
the cause and selecting appropriate treatment. And
because vomiting by itself can be a symptom of a
serious problem, it is usually important that we do
some tests and be proactive.
Vomiting with diarrhoea makes life a bit easier
because it is far more likely to be a stomach bug.
A history is very important at the
start. Importantly, how long has
the vomiting been occurring and
is it directly related to eating or
drinking. For instance, it isn't
uncommon for some cats to vomit
once a week or fortnight and be
eating well otherwise. As opposed
to a dog that has suddenly
started vomiting several times a
day and immediately after eating
or drinking. Also the age of the
patient is important.
An unvaccinated 10-week-old
depressed vomiting puppy that
has also developed diarrhoea is going to need
immediate aggressive treatment. Whereas a two-
year-old depressed labrador is likely to need an
X-ray to rule out a foreign object (ie four-legged
Vomiting is caused by either primary or secondary
causes. Primary causes are those that affect the
gastro-intestinal tract (stomach and intestines) like
infections or blockages.
Secondary causes are those as a result of an
abnormality or disease of another body system. For
example, pancreatitis (inflamed pancreas especially
dogs after eating an unusually fatty meal, such as
ham bones!) or kidney or liver diseases Where
do we start? A full physical exam, including the Dr
Doolittle thermometer is the starting point.
A blood test will eliminate a lot
of the secondary causes, which
is important to do. An X-ray
may find any obvious foreign
bodies. It is important to know
the limitations of X-rays though.
A lot of objects just won't show
But back to little Indy and the milk splatter. She turned
out to have pancreatitis caused by the infamous ham
bone (Ian is a chef). So three days on intravenous
fluids and no food along with IV medication and she
was back home to a happy family. Tip: Dogs generally
don't tolerate pork. So when you ask "what is causing
the vomiting?", be prepared for
a long list of possibilities.
